By selecting yes and signing my name below, I am agreeing and affirming that I, on behalf of myself and any other residents at the unit specified below, have read and understood the terms and conditions of the SPECTRUM TV PLATINUM and SPECTRUM INTERNET plan (the “Plan”) that is attached to this Spectrum Bulk Internet & Video Services Opt-In Request Form (this “Form”). I further understand that by executing this Form, I will be submitting payment for the Plan directly to Sun Communities, Inc. (“Sun”).
I agree that Sun shall not be held liable or responsible for any disturbances or damages that I may incur in connection with any service offered under the Plan. I also agree that I shall reimburse Sun for any damages that Sun may incur arising out of or relating to my use of the services offered under the Plan.

Spectrum Monthly Amenity Fee Internet & Video Services Include:
- Spectrum Ultra Internet: 400/20 Mbps (No Data Caps)
- Spectrum TV Platinum: 200+ HD Channels; Spectrum TV APP, 50K On-Demand Choices
- 2 Set Top Video Boxes
- 1 Internet Modem
Monthly Amenity Fee (charged to resident ledger the 1st of each month): $62.00
This agreement is an addendum to the lease, the monthly amenity fee is in addition to the monthly rent payment. Charge is considered additional rent, subject to late fees for nonpayment and collectible as rent in summary proceedings to recover possession of leased premises. Services are subject to cancellation and/or shut-off due to non-payment.
This amenity fee agreement will terminate upon termination of residency; or, if a 30-day written notice is submitted to the community office wishing to end amenity services. If resident wishes to terminate services, all equipment must be returned to Spectrum or additional charges will apply.
